    Nurse Navigator assists cancer patients/care givers on their journey through cancer care as an advocate, interpreter, educator, and counselor for oncology patients and their care givers. The incumbent serves as the primary point of contact and is responsible for coordinating the care and services of the identified patient populations for an extended period across the continuum of care. The incumbent promotes effective resource utilization and assumes a leadership role with the multidisciplinary team to identify and eliminate barriers to care to achieve an optimal clinical outcome for the patient and their caregivers.

    Qualifications

    In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:

    • Associate degree in Nursing (or Diploma), required.

    • Bachelor degreen in Nursing, preferred.

    • Registered Nurse (RN) license from state of employment or RN license from state participating in national license compact, required

    • Nursing Professional Certification, preferred.

    • 5 years' experience in oncology nursing in the inpatient and/or outpatient setting preferred

    • Experience/knowledge in community health setting/services preferred.

    Physical Requirements

    Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10

    lbs. constantly
